TELEHEALTH CONSENT STATEMENT

Community Stoma Service – Kelly Taylor, RN

By booking or participating in an online consultation with Community Stoma Service, you acknowledge and agree to the following:

1. Nature of Telehealth Support

Consultations are delivered online via video and are intended to provide education, self-management guidance, and emotional support related to stoma care.
These sessions do not include diagnosis, clinical assessment, or medical treatment.

2. Limitations of Telehealth

You understand that telehealth may have limitations, including:

  • technical issues or interruptions

  • reduced ability to visually assess certain concerns

  • the need for in-person medical review if symptoms worsen or require clinical intervention

If Kelly identifies that your situation requires medical attention, you may be referred to your treating GP, surgeon, stomal therapy nurse, or emergency services.

3. Privacy & Confidentiality

Your personal information will be handled in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).
While secure platforms are used, no online system can guarantee absolute confidentiality. You acknowledge the small inherent risks of communicating over the internet.

4. Emergency & Escalation

Telehealth sessions cannot be used for emergency care.
If you experience urgent symptoms including severe pain, active bleeding, signs of infection, stoma blockage, or sudden illness, you must seek immediate care by calling 000, visiting your nearest emergency department, or contacting your treating healthcare provider.

5. Accuracy of Information

You agree to provide accurate and complete information during your session so that guidance can be personalised appropriately.

6. Consent to Proceed

By booking or attending the session, you provide consent to receive stoma support through telehealth and accept the limitations, privacy considerations, and responsibilities described above.
